Hi Chris,

The ceramic bypass caps are all 0805 SMT, 0.1uF.

The LEDs are 0805 SMT.

The resistors are all 0805 SMT with values:
On -12, +12 supplies: 1k
On -5, +5 supplies: 330
On +3.3 supply: 150

The tantalums are 6032 SMT, 10uF.
