We might be able to arrange a group buy and supply the PCB and the 
hard-to-get parts.  To get the parts pre-mounted is a greater 
complication due to RoHS regulations.

I suspect the parts would be:

1) AK5394A
2) DS2431P
3) EPM240T100
4) LT1128 (note that other, less expensive and more available op amps 
may be as good in this application)

There would still be the issues of importation and taxes to be paid by 
the purchaser of the board and parts.
