Not sure if this will help; the LTC1992 would seem to meet your criteria 
- fully differential, rail-rail I/O, low power consumption, adjustable 
output common mode voltage, 2.7V single ended supply, low cost etc etc.

p29 of the data sheet has an application example for a 3-pole low-pass 
filter with all the needed equations.

I think we might be using this on the uWSDR project.
