The clearance fee is common with all carriers in my experience. My
experience here in the UK with carriers is:

O FedEx deliver then send you a bill for VAT, duty and fees separately by
mail;
O ParcelForce automatically hold the parcel if there are any fees payable
(they used to attempt delivery);
O UPS attempt delivery three times then hold it unless you can pay the fees
COD or over the 'phone (see below);
O DHL deliver then I hear nothing about any fees.

For UPS the trick is to track the package, and just as it clears customs
call them up and pay the fees and they will give you a reference that you'll
need to provide the delivery driver because their system is not joined up.
Both UPS and ParcelForce have occasional black holes where a package will
stick for days on end and then suddenly it will move. In those
circumstances, don't expect to speak to anyone who'll be able to give you
anything more than lip service.
