[hpsdr] Hermes FPGA source code

I compiled this today using Quartus II 12.0sp2 web edition, uploaded
it to Hermes and it runs successfully, at least the receiver runs
OK with ghpsdr3-Qt - I haven't yet tested other features. I do get
quite a few warning messages during compilation though. I did try
to set the compilation options as described in Hermes.v but some
of them are different in 12.0sp2.
