Thanks to Brian for his articulate explanation.  I love to read his stuff!  To add one point: the two-tone test results that are viewed on the screen reflect what the radio's processor sees, not necessarily what is going to the antenna.  In other words, if the radio's internal leakage is greater than the sampled RF from an external amplifier, then that's mostly what you will see on the screen.  You won't see whatever distortion is being added by the amplifier.  Therefore, you may think that you have terrific IMD performance when in fact it's not so good at all.
